Submit the contact form on the FAQ page. We'll email you a donation release form, mailing address, and step-by-step instructions. Be sure your email is correct—and check your spam folder if you don't hear from us soon.
Submit the contact form on our site to receive a link. Print and include the bottom portion of the form with your dress. Keep the top portion for your tax receipt.
Please check your spam or junk folder. If you still don’t see our email within a reasonable time, reach out again to ensure your email was entered correctly.
We don’t publicly list our mailing address on the website to help us manage donations efficiently. Please submit the donation request form first, and we’ll email you everything you need—including the mailing address, detailed instructions, and the required donation release form. Please do NOT send with “signature required.”
Thank you for understanding and helping us stay organized!

Absolutely! Visit EveFoundation.org/shop to place your keepsake order before mailing your dress. Add your order number to the release form so we can match your keepsake with your dress. Packs ship within 4–6 weeks of receipt. Our Keepsake program provides 90% of our funding.
If your combined donation and/or keepsake order totals $100 or more, you're eligible for a complimentary locket.
Be sure to mark your preference on the release form.
That depends. Please consult your tax professional to determine eligibility. In the case that it does qualify, the top portion of your release form serves as your tax receipt.

No, we’re not able to provide individual photos. We receive a high volume of dress donations and our volunteer seamstresses devote their limited time to carefully creating the garments for families. Photographing, labeling, and emailing photos for each dress would slow production and delay deliveries to hospitals
